- Online Questionnaire

All of our patients under going invasive surgery need to complete a questionnaire pre procedure and at pre set times afterwards. Historically we have sent these out by post but it is now possible to fill these in online. Unfortuantely they have not been fully automated so after completion they will need to be returned by email (contact@thebackdoor.org.uk) as an attachment. If you are unsure of how to do this, or are unhappy to send your information online for security reasons, please request a copy by post (by phone or on the above email) or print out the form and send it to the following address:
